Tax Return Checklist

Business Activity Statement (BAS) Information Form
If we are preparing your BAS on your behalf, this form helps us capture the figures and transactions we need. Complete it each quarter and return it ahead of your lodgement deadline.
Authority to Act (ATO Representative)
This form authorises GFM Accountants to communicate with the Australian Taxation Office on your behalf. It is required before we can access your ATO records, lodge documents, or respond to correspondence for you.
SMSF Annual Return Information Form
For self-managed super fund trustees, this form collects the financial and member information needed to prepare your annual return and financial statements. Please complete it in full before your scheduled review.
Change of Details Form
